Summary of Novel Navigated Bedside EVD Insertion Technique

What’s New: A new technique for bedside external ventricular drain (EVD) insertion is introduced, utilizing a mobile health-assisted navigation instrument (Thomale Guide) and a battery-powered single-use drill (Phasor Health). This method aims to improve the safety, accuracy, and efficiency of EVD placement in emergency neurosurgical settings.

Importance: Traditional freehand EVD placement often results in malpositioning, leading to complications. This novel technique could potentially reduce such risks and improve patient outcomes.

Contribution to Literature: The study presents clinical case series data from 12 patients (8 female, 4 male) who underwent the navigated bedside EVD procedure. All EVDs were successfully placed at the first attempt around the foramen of Monro. The mean operating time was 25.8 minutes (±15.0 minutes), with no revisions needed for malpositioning or dysfunction. The average drainage volume was 41.3 ml per day (±37.1 ml), and the mean EVD stay was 6.25 days (±2.8 days). Complications were minimal, with one postoperative subdural hematoma and one cerebrospinal fluid infection.

Results: The technique proved to be effective in the studied cases, suggesting that it could be a valuable addition to emergency neurosurgical procedures.
