This registry study evaluates the Silk Vista “Mama” (SVM) flow diverter’s effectiveness and safety in treating proximal intracranial aneurysms. Data from nine Italian neurovascular centers, covering August 2020 to March 2022, included 48 aneurysms in 48 patients. The findings highlight that most aneurysms were small (≤10 mm, 60%) and unruptured (65%), with 77% involving the internal carotid artery. The SVM flow diverter showed a high technical success rate, with optimal device opening and wall apposition achieved in 96% of cases. There were few intra- or periprocedural complications, none leading to new neurological deficits. Importantly, no flow diverter-related morbidity or mortality was reported. At mid- to long-term follow-up, complete aneurysm occlusion was achieved in 76% of cases, with 88% showing complete occlusion or entry remnant. This study underscores the SVM flow diverter as a safe and effective option for proximal intracranial aneurysms, with low complication rates and satisfactory occlusion outcomes.
