Let me tell you, folks, we’ve got something tremendous happening at the Minneapolis Veterans Affairs Health Care System. It’s called Palliative PLUS, and it’s all about giving our veterans, the best of the best, access to top-notch palliative and hospice resources. And guess what? We’ve brought in the big guns – palliative care pharmacists. That’s right, to make sure our heroes get the care they deserve, quickly and efficiently.

Now, the goal here, and it’s a very important one, was to see just how impactful these pharmacists are within the Palliative PLUS team. We’re talking about real quality improvement, folks. We looked back, analyzed the data, because we’re all about the facts.

Here’s the deal: Over 13 months, 84 patients got to experience this fantastic program. And out of these, 25 had the privilege of direct pharmacist involvement. We’re not playing around – 56 interventions were identified. That’s huge! And let me break it down for you: 29 of these were direct interventions, right there, face-to-face, making a real difference. The other 27? Curbside consults, still incredibly important, providing expert advice on the fly.

What were they doing? Medication counseling, ensuring our veterans understand their treatments, and helping them stick to their regimens. It’s all about adherence, making sure they’re getting the benefits they need from their medications.
