The Australian Traumatic Brain Injury Initiative (AUS-TBI) is working on a data resource to enhance outcome predictions for patients with moderate-severe traumatic brain injury (msTBI). A key component of this resource is a data dictionary, which was developed through a systematic review and consultation process to identify acute interventions that could alter clinical outcomes for msTBI patients.
The review involved a comprehensive search of bibliographic databases up to April 2022, focusing on English-language reports of randomized controlled trials (RCTs) with at least 100 msTBI patients. The studies examined the relationship between acute interventions and clinical outcomes, using a predefined algorithm to evaluate the strength of these associations.
Out of 14,455 records, 124 RCTs were screened, and 35 studies were included, assessing 26 unique interventions and 21 clinical outcomes. The review identified four interventions with medium modifying value: tranexamic acid and phenytoin (positive effects), and decompressive craniectomy surgery and hypothermia (negative effects). Through consensus, an additional eight interventions were selected.
Importance: This work contributes to the AUS-TBI’s goal of improving msTBI outcome predictions by identifying 12 interventions with potential modifying effects on clinical outcomes, which will be included in the national data resource.
